For the 2014-2015 school year, ALL Harriman Middle School students will be receiving breakfast and lunch at no cost, regardless of family income.  Please see this form for information about this program.


Over the Counter Medicine Form 8/5/2014

If your child is allowed over the counter medication, this form mustbe filled out and returned to the school along with the medication he or she is allowed to take.  Harriman Middle School will notprovide over the counter medication of any sort to students.  Medication must be sent to school in the original, unopened package with the student's name on it.


Prescription Medicine Form 8/5/2014

If your child requires prescription medication while at school, this form must be filled out, and the prescribing physician must sign the form.  This form must be turned in to the school along with the student's medication in the original prescription packaging.
